It is expected that deductibles, co-payments, and other amounts not covered by insurance will be paid prior to services being provided. You will be contacted prior to your procedure with an estimate.  Accepted forms of payment include personal check and credit card (we do not accept AMEX). Payment may be made over the phone prior to your procedure or at check in. If you have questions at any time, let us know.
